How long has it been happening? What information may still be missing? Inspect a component or part of the system
What has already been tried? What are the most reasonable possible causes? Change an operating or control condition
What are you least confident about? What should be checked before money is spent? Repair, test further, or evaluate equipment only if the evidence supports it
